1️⃣ Application Form

• Complete the official application form with your personal and professional details.

• Submit along with the required fee.

2️⃣ Acupuncture Form 2A – Preprofessional Education Verification

• Section I (To be completed by the applicant): Fill out this section before sending the form to the school(s) where you completed your general/preprofessional education.

• Section II (To be completed by the school): The Registrar must complete and send this form directly to the Office of the Professions at the address listed on the form.

3️⃣ Acupuncture Form 2B – Professional Acupuncture Education Verification

• Section I (To be completed by the applicant): Fill out this section before forwarding the form to the school(s) where you completed your acupuncture education.

• Section II (To be completed by the school): The Registrar must complete and submit this form directly to the Office of the Professions.

4️⃣ Acupuncture Form 3 – License/Certification Verification

• Must be submitted directly by the licensing or certifying authority (e.g., another U.S. state or foreign licensing agency).

• The Office of the Professions (OP) requires that this verification is received without the applicant’s access or intervention.
